Community Health Workers (CHW)

About Us

Building California's Trusted Community Health Workforce

Life Coach Navigators is an official workforce training academy and infrastructure hub designed to recruit, certify, and deploy credentialed Community Health Workers (CHWs). We transform local lived experience into highly specialized professional expertise. By anchoring our curriculum in state-mandated core competencies, we equip our trainees to dismantle structural barriers to care, navigate complex health systems, and serve as certified Lead Care Managers within California’s CalAIM framework. We bridge the gap between vulnerable populations and sustainable community health delivery.

Who we are

We are a specialized workforce development pipeline dedicated to training the next generation of public health leaders in Oakland and the greater Bay Area. We understand that sustainable community health requires a thoroughly trained, culturally authentic workforce. Our academy takes passionate individuals from the community and equips them with a standardized, multi-disciplinary toolkit. Through rigorous curriculum alignment, clinical oversight, and professional development, we prepare our graduates to step seamlessly into managed care networks and community-based organizations to build health equity from the ground up.

What We Do

We deliver a structured, comprehensive training ecosystem that satisfies California's strict credentialing pathways for Community Health Workers. We design and administer curriculum models that address the root causes of health disparities—the Social Determinants of Health (SDOH). By providing rigorous instruction on complex case management, trauma-informed outreach, data compliance, and healthcare navigation, we build an enterprise-grade workforce ready to meet the operational demands of regional health systems and state-funded capacity initiatives.

How We Do It

We execute our mission through an intentional, dual-track training framework that perfectly aligns with California Department of Health Care Services (DHCS) standards:

  • The 80-Hour Core Competency Pathway: A robust, evidence-based curriculum covering the eight state-mandated domains, including motivational interviewing, screening and assessments, crisis management, and system navigation.

  • Supervised Field Practicums: Trainees participate in rigorous, non-billable field placements, clocking active field hours under strict clinical supervision before transitioning into independent, billable frontline roles.

Why We Do It

Modern healthcare requires a person-centered, community-led approach. By formalizing CHW education, we ensure that managed care plans have access to an audited, PAVE-ready workforce capable of managing high-risk Populations of Focus, driving down unnecessary emergency room utilization, and improving long-term clinical outcomes.

  • 80+ Hours: Standardized Core Competency Training Layout

  • PAVE-Linked: Streamlined Provider Profile Integration Readiness

  • CalAIM Aligned: Purpose-built infrastructure for Enhanced Care Management (ECM)

Our Specialized Academy Offerings

Workforce Training & Certification We provide comprehensive instruction mapped to California’s CHW medical benefit guidelines. Our curriculum explicitly trains workers in structural advocacy, chronic disease navigation, and housing transition services, ensuring they are fully prepared to log accurate, reimbursable medical encounters.

CalAIM & ECM Infrastructure Alignment We partner with local Community-Based Organizations (CBOs) and Managed Care Plans to scale up their frontline staffing. Our trainees are explicitly educated on healthcare billing workflows, standard electronic data documentation, and compliance frameworks—allowing organizations to seamlessly utilize G-codes (G0019/G0022) and draw down sustainable Medi-Cal revenue.

  • Data Privacy & HIPAA Security: Our corporate leadership maintains a strict data-security layout. All trainees are fully instructed on secure Electronic Health Record (EHR) entry, encrypted data management, and federal HIPAA compliance, protecting member privacy across all digital health ecosystems.
